Objective:To study based FMEA hemodialysis device fault management. Methods:Using FMEA as a research tool, study hemodialysis equipment failure information. For the consequences of potential failure modes and failure to produce, analyze their severity, cause of the malfunction, the frequency of occurrence and detection, referring to the evaluation criteria for risk assessment. Results: By comparing effect, the report of equipment FMEA was obtained,summed hemodialysis equipment including machines in self-test, treatment and disinfection, such as different operating conditions common failure mode. Conclusion: For used in fault management of hemodialysis equipment, FMEA plays an important role in the equipment preventive maintenance.

Based on the shortcomings of traditional FM EA in analyzing problems and finding solu-tions ,an innovative design method integrated with fishbone diagram ,FMEA and evolution laws was proposed .This method used the hierarchical analysis of fishbone diagram to assist FMEA fully excavate possible failure modes ,then analyzed failure modes according to the standard ex-pression of FMEA and determined the Risk Priority Number (RPN) .By adopting evolution laws , the method can solve the failure problems effectively and efficiently .T his paper also verifies the feasibility of this integrated method with an example of vibration damping design on rail track .

Track circuit is the key equipment of railway signalling systems,and its reliability is ex-tremely important to ensure the safety of railway transportation.On the basis of FMEA theory,a FMEA analysis method for ZPW-2000A Jointless Audio Frequency-shift Modulated Track Circuit system is proposed.The corresponding FMEA content,the division rules of analysis level and fail-ure criterions are defined according to the characteristics of track circuit system,and the confir-ming method of severity,occurrence and detection is also discussed,which is used to calculate the risk priority number (RPN ).Based on the actual working condition of ZPW-2000A Jointless Au-dio Frequency-shift Modulated Track Circuit system,the FMEA analysis is conducted.The re-search indicates that the FMEA method can identify the potential hazards and the weak links which affect system reliability.It provides a practical method and theoretical basis for the reliabili-ty research of ZPW-2000A track circuit system.

Objective: To investigate the application results of failure mode and effects analysis (FMEA) in central sterile supply of cleaning equipment. Methods:The FEMA team were established from January 2010 to December 2013.FMEA application mode in sterile supply room equipment cleaning process,and analyze the supply room management quality score before and after the implementation of FMEA risk event occurs, the department nurses and doctors cleaning equipment satisfaction. Results: The RPN values of completely rinsed , rusty equipment , cleaning process errors, not timely processing equipment, precision instruments, loss, failure to step after implementation were decreased than previous implementation.Conclusion: FMEA model can effectively improve the sterile supply room management quality and reduce the risks involved in cleaning equipment , cleaning equipment to improve quality .

Objective To discuss the application effect of FMEA in reduction of unplanned extubation of PICC.Methods FMEA was applied to find out the failure mode of unplanned extubation of PICC,and then assessed it from three dimensions,which were the likelihood of occurrence,the possibility of being discovered,and the seriousness of the failure,and then calculated the RPN values,risk priority,and plan optimization process,at the last made a proactive behavior intervention.81 patients were selected as the control group before the application of FMEA and 98 patients were set as the experimental group after the application of FMEA.The incidence of unplanned extubation was compared between two groups.Results There was statistically significant difference in unplanned extubation between two groups.Conclusions Application of FMEA can reduce the rate of unplanned extubation of PICC.

Hydraulic system has been widely applied in high-power and automated construction machinery.But there are many difficulties in its fault diagnosis because of the complicated struc-ture of hydraulic system.Especial y in the working site,it may cause the shutdown of work when the fault can’t be removed timely.In this study,an extended failure mode and effects analysis (FMEA)method is presented to integrate expert knowledge and working data.A remote condi-tion-based maintenance approach is presented,and an application system with FMEA knowledge base has been developed for construction machinery hydraulic system.The system structure and some key problems such as data acquisition,fault features extraction,as wel as FMEA knowl-edge base is discussed.An application example shows the system is content.
