全科医学在我国经过近二十年的探索和实践,取得了长足发展,但全科医学的宗旨尚无法在社区卫生服务中得到完全的实现。因此,如何缓解卫生保健的供需矛盾,提升初级卫生保健能力,实现全科医学的宗旨,已成为当前社区卫生服务需要思考的问题。结合当前社区卫生服务中心的现状以及我国医学教育的特点,我们认为,加强中医全科建设,是提升初级卫生保健能力、满足群众基本医疗服务需求,实现全科医学宗旨的一种积极探索。

Although general practice medicine has been developed for twenty years and made great progress in China, its aim is still not fully achieved in community health service center. The ifrst thinking should be how to alleviate the supply and demand contradictions and how to achieve the aim of general practice medicine. Considering the present situation of community health service center and the characteristic of our medical education, we believed that strengthening the general practice of traditional Chinese medicine is a new way to improve the primary health care and meet the basic medical demand of people and then achieve the aim of general practice medicine.

英文中的基础医疗卫生服务（ Primary Health Care，PHC）被中文错误地理解和翻译成“初级卫生保健”已有半个多世纪。这个错误在我国政府和医药卫生领域产生了一系列误导，例如在国家医药卫生发展的整体方向和布局上，在政府医疗卫生政策的制定和人才培养上，在国家医药卫生财政拨款的方向上都产生了负面影响。本文从英文语言单词本身的分析，从医药卫生学术领域的理论和内涵，从世界各国医疗卫生服务的实践经验，以及对我国医药卫生事业产生的不利影响等多个角度，阐述中文“初级卫生保健”对英文Primary Health Care 的误译和对这个概念本身的完全误解。在我国进入新一轮医药卫生改革，发展我国基础医疗卫生服务事业，建立基层医生制度的今天，希望引起我国医药卫生学术界和我国政府的卫生行政部门重视，尽快纠正这一概念误译，正本清源，以促进我国新一轮医药卫生改革健康发展。

近年来英国将卫生服务与社会服务体系进行整合,各地在对整合型保健进行探索与试点过程中产生了一些典型做法,并取得了较为明显的成效。本文主要通过现场观察与深度访谈的定性研究方法,以英国牛津郡对整合型保健服务的探索为典型案例,对其精神卫生服务保健方案以及社区急诊多学科团队的服务方式进行了分析。研究认为英国整合型保健将初级卫生保健和社区卫生服务作为卫生体系的核心,与其他社会服务进行整合,其多学科服务团队、整合型的服务路径、个性化服务方案、强调病人参与和自主等做法为我国医改过程中卫生服务体系的改革提供借鉴。

An integration of British healthcare and social services has been underway in recent years, and a typical methodology has been formed in the process of regional explorations . Using qualitative research methods of on-site observation and in-depth interviews, the paper studies the Care Programme Approach ( CPA) system of mental health and emergency multidisciplinary teams of Oxfordshire, a typical case of the integrated healthcare system re-forms in the British healthcare system. England’s integrated healthcare reforms have placed primary care and commu-nity health services at the center of the healthcare system and have included social services within the system. The ex-periences of multidisciplinary teams, integrated care pathways, personalized care planning and patient involvement and independence from the reforms are worthy subjects of study for China’s own healthcare delivery system reforms.

目的：研究提出医疗服务价格项目定价的框架，并根据提出的框架系统介绍新版医疗服务价格项目权数计算方法。方法：根据新版医疗服务价格项目指导手册，借鉴美国卫生保健财政管理局（HCFA）和美国医学会（AMA）的“以资源成本为基础的相对价值（RBRVS）”研究成果，提出医疗服务价格项目分析框架和维度，所介绍方法为国际权威的医疗服务价格定价研究方法。结果：基于各组成要素的百分比和各专业特点，通过引入权数计算模型，构建医疗服务价格项目权数计算模型，依据该模型计算出各项目的权数。结论：可以按医疗服务价格项目不同维度开展医疗服务价格构成因素权数计算研究，实现新版医疗服务价格项目定价工作，更好地为医药体制改革服务。

Objective: To develop price framework of medical service price project and introduce the weights calculation method of the new edition medical service price project. Methods: According to the new instruction manuals of medical service price and the research of the resource costs based on the relative value(RBRVS)of the U.S. Health Care Financing Administration (HCFA) and the American Medical Association ( AMA ) , an analysis framework and dimension of medical service price was raised and authoritative international medical service project price methods w ere summarized. Results: Based on the percentages of constituted factors and the professional features, it found the weights calculation model could be constructed with the weights model, and it could calculate the weights of the elements.
